School Building Use Request Form

The majority of PTA sponsored activities that occur in the school after/before the school day require you to complete Form ADM-20 to request use of the school. This is what we use for Multicultural Night, Holiday Shop, Art & Dessert, PTA Meetings, etc.  If you have multiple events, you can complete one form but list the multiple events on an attachment as long as each question on the form is answered in the attachment.  For a recurring event, only file one form and insert the multiple dates on the form or in an attachment.

For a PTA sponsored after school class or activity that charges a fee to participate and is being held in the school, you need to complete Form ADM-24 to request use of the school.  Anyone using ADM-24 should consult with Monica Cramer (the PTA after school program representative) before submitting so she can assist you with the various nuances of the form and any additional paper work that may be required. If the activity does not meet on school property, the form is not required.

These forms will be used to put all PTA events and meetings on the school master calendar and the building use calendar, pending school and FCPS approval.  If you have any questions regarding form completion, please contact Monica Cramer.
